Neighborhood Properties, Inc. receives referrals for housing from the Toledo Lucas County Homelessness Board through a Coordinated Entry process.
To be initially eligible for the Coordinated Entry process participants must:
- meet the HUD definition of homeless – living in a park, a car, abandoned building, etc.…
- be currently working with a community mental health agency.
Please contact your caseworker for assistance with the process.
If you meet the definition of homeless and have worked with a community mental health agency in the past, you will have to reconnect with that agency or another for assistance with the process.
If you meet the definition of homeless and have never worked with a community mental health agency:
- Please call us at 419-473-2604 and ask for a PATH outreach worker. NPI has a team of outreach workers that will be able to assist in linking you with an agency and walk you through the Coordinated Entry
